Burlington (VT) Group Purchases Van to Provide Medical Care to Homeless

According to a report from VTDigger, the Community Health Centers of Burlington (CHCB) announced that it has purchased a new outreach van to expand access to services provided through their Homeless Healthcare Program (HHP).

When the COVID-19 pandemic first hit, and CHCB began offering an outreach testing site in partnership with the Vermont Department of Health, the Center was loaned a van by Champlain Housing Trust. Now, thanks to federal funding in support of expanded coronavirus testing, it is now possible for CHCB to buy a van of its own.

The van will allow CHCB to expand medical outreach visits to include acute and chronic disease management and preventive care, immunizations, care coordination, connection to mental health services, and COVID-19 testing when clinically indicated. The CHCB will also use the van as part of its Youth Program to provide care and support to at-risk or homeless youth ages 15 to 26. This model of care will revolutionize the way health care is provided to Burlington community’s most vulnerable people.
