Survey to Look at Motivating Factors Behind Vaccination Decisions in EMS

Your Input is Needed

This survey seeks to obtain the opinions of active emergency medical providers on the topic of COVID-19 vaccination.

Participation in this survey entails the completion of a short anonymous questionnaire regarding participant demographics and positions towards COVID-19 vaccination. This survey is completely voluntary; questions can be omitted, and the survey can be ended at any time. Data will be de-identified and kept confidential in a password protected file available only to the researchers involved. This survey has been approved by an IRB.

This survey is designed for emergency medical service providers (over the age of 18) including but not limited to:

– EMTs
– Firefighters
– Paramedics
– Ambulance Operators
– Ambulance Attendants
– Law Enforcement Officers
– Volunteer and Career Workers
– First Responders Providing Emergency Medical Care
