    Northern California Man Set on Fire While on Public Bus LODD Report Released on Virginia Medic’s Fatal Fall ALEXANDRIA, Virginia – The Alexandria Fire Department has released the line of duty death investigation report involving Medic II Joshua Weissman. Read the Report The investigative report compiled a list of 19 factors identified as a comprehensive human error framework. The fire department’s Health and Safety Unit addressed the contributing factors that may or may not have led to the fatality. They are: 1. Accountability 18. Unknown 19. Weather The report looks at some of the above causal factors as they relate to this incident. 11 recommendations were developed based on the investigation findings. Each one offers various next steps in order to prevent another similar fatality from occurring.
