Dear EMS Professional,

A few weeks ago the Centers for Disease Control and Prevention (CDC) invited you to participate in a survey we are conducting with EMS professionals like you to help us better understand the impact and implementation of the Field Triage Decision Scheme: The National Trauma Triage Protocol. If you have already taken this survey, we thank you for your input. If you have yet to fill out the survey, we would greatly appreciate it if you would. We urge you to take this opportunity to share your thoughts and opinions with us. Time is running out for you to take the survey, it will close on September 30, 2010!

What you think really matters! Responses to this survey will help guide future updates to the Field Triage Decision Scheme and related educational programs over the next few years. These updates relate directly to your daily work to respond to injuries and save lives.
