Staten Island EMT Among 4 Arrested for Forging Doctor's Notes

NEW YORK -- A Staten Island EMT was among four city employees arrested on charges of submitting fraudulent forms to the Fire Department to excuse absences from work.

Edwin Ortiz, 40, was charged with two counts each of tampering with public records and criminal possession of a forged instrument following his arrest June 10 as part of a city Department of Investigation probe.
